However, the most common cause of anal pain is actually an anal fissure. An anal fissure is a tear in the anal lining worsened by a spastic anal sphincter muscle. Treatment is based on relaxing the spastic muscle either with medicine or surgery. Many fissures will be healed simply with ointments we prescribe. If surgery is needed our physicians can perform a sphincterotomy (cutting the muscle) or even inject botox. As we like to say “Botox for the Buttocks”
